Bring your passion for Behavioral Health to Banner Thunderbird Medical Center! Our Behavioral Health Unit is made up of 56 beds and cares for a wide range of different mental health diagnoses as well as those behavioral health patients whom have co-morbidities that require an extra level of acute medical care.

As a Behavioral Case Manager you will have the opportunity to work with a wide variety of diagnostic categories and age groups. Our case managers are scheduled to work 5 consecutive days and we provide 7 days a week coverage. This role will consist of a Monday to Friday schedule with occasional weekend coverage. The psychiatrists, nurses, and Behavioral Health case managers work closely as a team to strategize the treatment approach. Beginning May 2021, there will be ample opportunity to practice your family therapy skills with adolescents, and group therapy skills and addiction counseling skills with our adults. Ours is a fast paced environment that is never dull or routine. Nonprofit Banner Thunderbird Medical Center opened its doors in 1983 and has been growing ever since to meet the health care needs of the communities it serves in northwest metropolitan Phoenix. Today, the 555-bed hospital - one of the largest hospitals in the state - is annually ranked as one of the top hospitals in the Phoenix metropolitan area by US News & World Report and enjoys a reputation as the preferred provider of care to children and adults in the area in consumer preference surveys. The tertiary care hospital provides everything from routine to advanced, specialized care to both adult and pediatric patients. Specialized services include cardiology, orthopedic surgery, pediatrics, neurosurgery, robotic surgery, stroke care and a women and infants services program. Additionally, in partnership with top-ranked MD Anderson Cancer Center, Banner Thunderbird also offers state-of-the-art cancer care. Banner Thunderbird is certified as a Joint Commission Primary Stroke Center and features a 35-bed Level III neonatal intensive care unit that cares for babies of high-risk pregnancies and those born prematurely. Banner Thunderbird is also home to the only pediatric intensive care unit for seriously ill or injured children in the area. POSITION SUMMARY

This position provides comprehensive care coordination for patients as assigned. This position assesses the patient's plan of care and develops, implements, monitors and documents the utilization of resources and progress of the patient through their care, facilitating options and services to meet the patient's health care needs. The intensity of care coordination provided is situational and appropriate based on patient need and payer requirements. This position is accountable for the quality of clinical services delivered by both them and others and identifies/resolves barriers which may hinder effective patient care.

CORE FUNCTIONS

1. Manages individual patients across the health care continuum to achieve the optimal clinical, financial, operational, and satisfaction outcomes.

2. Acts in a leadership function with process improvement activities for populations of patients to achieve the optimal clinical, financial, operational, and satisfaction outcomes.

3. Evaluates the medical necessity and appropriateness of care, optimizing patient outcomes.

4. Establishes and promotes a collaborative relationship with physicians, payers, and other members of the health care team. Collects and communicates pertinent, timely information to payers and others to fulfill utilization and regulatory requirements.

5. Acts in a leadership function to collaboratively develop and manage the interdisciplinary patient discharge plan. Effectively communicates the plan across the continuum of care.

6. Educates internal members of the health care team on case management and managed care concepts. Facilitates integration of concepts into daily practice.

7. May supervise other staff.

MINIMUM QUALIFICATIONS

Requires master's degree in social work or related field or equivalent experience. Employees working at Banner Thunderbird Medical Center must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment.

Requires a proficiency level typically achieved with 5 years clinical experience and as required by state law, a licensure in at least one of the following categories: social work, professional counseling or marriage and family. (Certification requires two years post graduate experience in a mental health setting or equivalent combination of education and documented supervised clinical experience.)

Positions providing direct services to substance abuse program patients must also possess board licensure in addictions as required by state.

Must have a working knowledge of care management, hospital and community resources.

Must demonstrate critical thinking skills, problem-solving abilities, effective communication skills, and time management skills.

Must demonstrate ability to work effectively in an interdisciplinary team format.

Employees working at Banner Behavioral Health Hospital or BTMC Behavioral must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment.

Additional related education and/or experience preferred.
