- 100% Outpatient Family Medicine with a state of the art FQHC - 4 Day Work Week with No call - OBGYN, Pediatrics, Family Practice, dental, behavioral health, MAT, vision, physical therapy, Podiatry, care/case management, referral assistance, translation services, free transportation and overall high retention rate and employee tenure at CHDC has contributed to an amazing, caring and smart team to make this a great place to work! - CHDC is part of Southeast HSX exchange which has thousands of other primary care members as well as hospitals so any patients records from hospital visits come into HSX and then into patient's charts so Providers have up to date information. - National Health Service Corp - Loan Forgiveness Lottery - Strong collaborations with local organizations, schools, local hospital to work together to improve community needs. - Guaranteed Base Salary with Full Benefits (Health, Dental, Vision, Paid Malpractice)
