You Will Be Responsible For:


  • Utilizes evidence-based interventions to assist patients, families, and significant others in coping with the psychosocial aspects of illness and its treatment.
  • Develops discharge plans in collaboration with physicians, nurses, physical therapists, nutritionists, administrators, educators, community agencies and others. Develops discharge plans that provide continuity of care based on the individual patientâ™s needs within 24-48 hours after admission.
  • Consults with physicians, patient, hospital staff, family members or significant others in a timely manner to facilitate resolution of identified barriers to treatment progress to ensure the continuity of care.

  • Maintains pertinent records of social work intervention, assessment, diagnostic impression and treatment for use in compiling accurate statistics.

  • Documents social work intervention in the medical record that reflects the current findings and status of the intervention at the time of the intervention.

  • Makes referrals to Federal, state, county and community agencies in a timely manner. Develops working relationship with agencies to ensure appropriate service delivery to patients.

  • Assesses patient for behavioral health needs as needed. Provides referrals for behavioral services as needed. Provides provisional diagnoses for mental health issues according to the DSM-V as needed.

  • Participates in daily multidisciplinary discharge planning meetings.

  • Provides referrals or consults with additional team members when identification of issues needs to be attended by the treatment team.

  • Articulates social issues during multidisciplinary meetings. Participates in and contributes to quarterly Ethics Committee.

  • Maintains up-to-date records on concurrent review patients.

  • Maintains effective communication techniques with physicians and staff members.

  • Knowledgeable of current Federal, state and other regulatory agencies in the areas of quality, performance, utilization and patient care.

  • Identifies opportunities for expeditious discharge planning and evaluates the patientsâ™ and familyâ™s receptiveness to a plan.

  • Ability to communicate effectively with parents when attempting to assess, diagnose, and treat infants and children. Ability to identify and discuss suspected and/or established abuse and neglect with parents.

  • Reports suspected abuse and neglect to Child Protective Services.

  • Exhibits empathy and acceptance of adolescent mood variance and attempts at independence.

  • Identifies social problems and financial limitations in patients that may contribute to discharge and treatment problems. Identifies and provides info about community resources from which patients may benefit.

  • Assist patient by providing information on, and completing when necessary, advance directive. Possess the ability to discuss end of life and geriatric assistance needs with patients.

  • Ability to recognize, identify, and communicate potential abuse and neglect issues with patients and family members. Effectively communicate with Adult Protective Services to ensure continuum of care during and after hospital discharge.

  • Communicates discharge planning options and assesses needs for long term care facilities. Advocates for the patientâ™s right to self-determination regarding treatment plans.

  • Provides education about discharge options and explores options with patients and members of patientâ™s care system. Identifies community resources that the patient may benefit from and communicates options with patients and families.

  • Maintains current Texas social work license and completes continuing education unit accordingly.

  • Remains current in BLS certification as required.

  • Develop and maintain internal and external contacts that enhance effective relationships for the hospital. Develops a working relationship with community organizations to establish maximum resources available to patients.

  • Attend conferences/workshops pertaining to departmental activities. Communicate effectively with team members of the Behavioral Health Center.

  • Compile timely resources and literature pertaining to departmental activities.

  • Maintain knowledge of current federal and state regulations and JCAHO standards.

  • Participate in hospital and community-based activities as the representative of the BHC of AdventHealth.

  • Initiates projects to improve the performance of the department and the care provided to patients.

  • Works independently by using critical thinking and creativity to incorporate evidence based social work frameworks.

  • Collaborates with other community agencies and organizations to improve knowledge and awareness of resources and services.



Qualifications

What You Will Need:
  • Requires graduation from an accredited masterâ™s degree program in social work, counseling, psychology or a related field, and appropriate licensure as required by state law. 

  • Requires knowledge of the principles, laws, and practices of social work in a psychiatric hospital setting

  • At least two (2) years of experience as a clinical social worker in a psychiatric hospital setting is preferred.

  • Experience should include individual, group, and family therapy and discharge planning in an acute care psychiatric setting.         

  • LMSW, LCSW, or LPC and the ability to complete and maintain certification in Crisis Prevention Institute (CPI) behavioral patient management course required.

  • BLS required

     
Job Summary:

AdventHealth Behavioral Health Center (AHHC) Clinical Social Worker position is a clinical position functioning within a multidisciplinary treatment team. The clinical social worker provides clinical mental health services for child, adolescent and adult patients and their families. Clinical responsibilities include conducting individual, family and group therapies; completing comprehensive psychosocial assessments; executing clinical case management tasks; and ensuring patients and family members receive quality care. This position reports to the Clinical Coordinator.
