As a Full Time 40 hours a week Nurse Liaison for ChristianaCare HomeHealth you will identify home care needs of patients to ensure a smooth transition from acute care and post acute care settings. The Nurse Liaison supports the intake process by interacting with discharge planners, case managers and patients within the acute and post acute care settings. The successful candidate will work out of our Avenue North office in North Wilmington.

The successful candidate will engage with clinical personnel who make referrals for home health care. The Nurse Liaison will be based in our Newark Campus, 5 days a week Monday through Friday 8am-5pm, working with other liaisons and unit staff. This person will be expected to acquire a minimum of 40 referrals a week and will work a weekend rotation - perhaps 1 out of 10, one holiday a year.

Requirements:

• RN must either possess a BSN or be willing to acquire within three years of start date
• Previous experience with HomeHealth is preferred
• Strong interpersonal and customer service skills
• Excellent communication and organization skills
• Ability to work and travel to hospital and post acute care settings
