The Autoimmune Brain Disease Clinic at Duke Children's is active in delivering exemplary care for each patient to ensure an excellent patient experience.
This position, of Autoimmune Brain Disease Nurse Navigator, will provide triage support and care coordination for this high acuity patient population in collaboration with dynamic multidisciplinary teams. This position will work closely with the faculty directors of Pediatric Neurology and Rheumatology. We are looking for someone with the ability to establish a strong rapport with patients and families by phone, coordinate services, and provide education and nursing support to patients and families in and out of the clinic setting.
The ideal candidate for this position will possess the following:
- A solid background in Pediatric Nursing; specifically neurology
- Experience with telephone triage or managed care coordination
- Strong oral /written communication skills, software navigation abilities (EPIC experience preferred)
- Good organizational skills
- Flexible attitude with the ability to function both independently and as a team member, assisting patients/ families through home care coordination and in the clinic setting
- Ability to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership and others
Work Performed
- Coordinate and lead the weekly ABD clinic care team meeting including agenda development, review of prior week's patient needs, upcoming weeks patient needs, review of new patient referrals , follow up on diagnostic testing
- Triage calls resolving problems within scope; gather relevant information and coordinate follow-up with providers to develop plan and convey plan back to patients. Attend clinic to meet and begin patient education with all new patients. Document communication in Maestrocare and manage inbasket messages regarding care.
- Field VDH pages and trouble shoot issues that may arise with patients receiving infusions; initiate all orders associated with infusion visits including IVIG, steroids, labs, other infusions.
- Coordinate peer to peer authorizations for medications and procedures including phone authorizations, literature review for evidence supporting use of therapy, drafting letters of medical necessity for therapies as part of the insurer's peer to peer process.
- Preorder labs for lumbar punctures and other procedures for both inpatient and outpatient; assure proper transport of lumbar puncture specimens to referral lab as needed
- Facilitate patient transfers; from outside hospitals to Duke, and from Duke to longer term facilities
- Coordinate renewal of outpatient prescriptions
- Round on inpatients with attending physicians as needed and facilitate coordination of care to outpatient.
- Coordinate new patient appointments assuring all necessary medical records, neuroimaging, and labs have been obtained from outside facilities
- Coordinate diagnostic testing and outpatient appointments here at Duke, or at an outside facilities
- Lead, develop, and standardize ABD training for care teams, develop order sets
- Consent patients for enrollment enter data into the ABD clinic registry and biorepository.
- Point person for coordinating ABD clinic programmatic growth; including updating website, track new developments in ABD market, liaison to Autoimmune Encephalitis Alliance and other organizations as needed
Work Hours & Shifts:
Full Time: Hours may vary, but will typically fall within a standard Monday-Friday, 8am-5pm schedule.
